Thank you for taking the time to complete this survey in regards to our LSWP. In accordance with state and federal regulations we are required to assess this policy a minimum of every three years. In order to include the community as much as possible the Wellness Committee is sending a survey to broaden stakeholder feedback.

 Our intention with the assessment is to look at the present Wellness Policy that we have in place and measure progress, identify gaps where policies need improvement, increase transparency with the policy, and work toward using the data collected to modify and strengthen the policy.
